Rules and Regulations

• This referral program is only applicable for current EVOS microscope owners (valid Serial Number is required and must be verified). Eligibility is at the sole discretion of AMG.

• Limit 1 redeemable product per referral.

• EVOS purchase by "the referred" must be verified prior to redemption.

• "The referred" must be new submission - customers already existing in AMG's database and/or who have been previously contacted by an AMG representative do not qualify as referrals.

• For EVOS owners in the U.S.

• The referred must be in the U.S. or Canada.

• Both "the referrer" and "the referred" will automatically be sent a unique promo code via the e-mails provided after submitting the referral. This code must be referenced and recorded with the new EVOS purchase, on the Purchase Order (PO) by "the referred."

• Please note: this code is only valid if all other terms and conditions have been met.

• "The referrer" identifies "the referred" individual by completing and submitting the form below. No other form of submission will be accepted.

• Unique EVOS serial numbers for both the "the referrer" and "the referred" must be verified by AMG.

• Name, shipping address, company/institution and PROMO CODE must match the referral form submitted by "the referrer" on the purchase order of "the referred."

• AMG reserves the right to change or discontinue the program at any time without notice.
